Clinical Week 12

Clinical Rotation –  Week 12 Journal

Dates of rotation: Monday – Thursday (June 25, 2018 – June 28, 2018)

Location: Harlem Hospital OB Clinic

Activities & Assignments:

Monday (8:30 am – 5 pm) 8 ½ hours

  • Staff huddle
  • Assessment and counseling of new OB patients
    • Thoughts: I really love counseling patients at the clinic. I find that nutrition during pregnancy is fascinating and helping pregnant women through their pregnancy journey is such a fulfilling experience.
  • Follow-up counseling of other OB clinic patients
    • Challenge: I find that 15 minutes is sometimes not enough for follow-ups since some patients still have a lot of questions and concerns.
  • Charting
    • Thoughts: I’m getting much faster at charting. The RD said that she usually only has 10 minutes to chart!

Tuesday (8:30 am – 4:30 pm) 8 hours

  • Staff huddle
  • Counseling follow-up high-risk patients
    • Thoughts: Most of the high-risk patients are getting transferred back to regular OB which is great news.
  • Continued charting previous day’s patient notes
    • Thoughts: We were a little behind on charting since Monday was very busy.

Wednesday (8:30 am – 4:30 pm; 7 pm – 8 pm) 9 hours

  • Staff huddle
  • HIPAA Training
    • Thoughts: Along with other interns at the hospital, we learned all about the importance of HIPAA and how important it is to protect patient information.
  • Edits for dairy and exercise handouts
  • Observed counseling of GYN patient
  • Charted GYN patient note
    • Thoughts: Putting together a meal plan for a patient is always enjoyable since it is such a personalized process.

Thursday (8:30 am – 5 pm) 8 ½ hours

  • Staff huddle
  • Counseling new OB clinic patients
    • A patient had more question about breastfeeding, so I referred her back to one of the nurses to get more information.
    • I asked one of the doctors to sign medical documentation for a patient to bring to WIC in order to get Ensure.
  • Patient follow-ups
